Lahir sebagai Diana Blanche Barrymore Blythe di New York City, New York, Diana Barrymore adalah putri dari aktor terkenal John Barrymore dan istri keduanya, penyair Blanche Oelrichs. Dia adalah putri tiri dari Dolores Costello, saudara tiri aktor John Drew Barrymore, dan Dolores Barrymore, juga bibi aktris Drew Barrymore. Dia memiliki dua kakak tiri, Leonard Jr. dan Robin, dari pernikahan pertama ibunya dengan Leonard Moorhead Thomas.
