1169年9月14日にマヌエル1世の妃マリアは男児を産み、生まれた子供はアレクシオスと名付けられた[38][39]。マヌエル1世はベーラと娘のマリアの婚約を解消し[38]、ベーラのデスポテースの称号を剥奪した上で下位の称号である「カエサル」を授与した[38][40]。1170年春にベーラはマヌエル1世の義理の姉妹であるアグネス(英語版)と結婚し[38][40]、二人は中東に聖地巡礼の旅に出た[40]。エルサレムに到着した二人は歓迎を受け、歓待の対価として聖ヨハネ騎士団に10,000ベザントを寄贈した[40][41]。ベーラが寄付に際して発行した書状には「Lord A., Duke of Hungary, Dalmatia and Croatia」という称号が記されていたが、マヌエル1世が授けたカエサルの称号は含まれていなかった[38]。
ハンガリーの使節団はマヌエル1世、セルディカ(現在のブルガリアのソフィア)のベーラの元を訪れた[45][37]。キンナモスの記録によれば、彼の兄弟が亡くなった後に「ベーラに正統性がある」ため「ベーラを王として派遣する」ことを求めたという。また、キンナモスはマヌエル1世が自身と東ローマ帝国のために「"promised an oath to observe for the whole course of his life whatever would be beneficial」上でベーラを王としたと伝えている。また、1196年に東ローマ皇帝イサキオス2世によって作成された書簡には、ベーラはセルビアが東ローマを攻撃した場合、彼らに援助を行わないことも約束していたことが記されている[45]。
